Consent to Treat a Minor – Chiropractic Care
This form grants permission for Dr. Mark Vettraino, a licensed chiropractor, to evaluate and provide chiropractic care to a minor (under the age of 18). Chiropractic services may include, but are not limited to, spinal adjustments, therapeutic exercises, soft tissue techniques, and other procedures within the scope of chiropractic practice as deemed appropriate for the minor’s condition.
