Our mission is to bring care to those who need it most—especially individuals who face barriers such as limited mobility, transportation challenges, or difficulty accessing traditional healthcare. We are deeply grateful to the communities that have welcomed us and opened their doors, allowing us to serve not only their residents, but also their families, friends, employees, and the surrounding local community. These partnerships allow us to extend our reach and fulfill our purpose of meeting people where they are.

While we spend one day each week at our Home Office, our care is primarily delivered within the following community locations, where ALL PATIENTS INCLUDING THOSE FROM THE SURROUNDING COMMUNITY ARE welcome to schedule an appointment.
